I have a permanent map of bad decisions etched into my skin, so trust me when I say Winston-Salem has gotten weirdly good at this. You can get a delicate fine-line piece from a patient hand at Seventh Sin, or you can stumble upstairs at Tattooz & Booz, where the music is too loud and the bar back saves your spilled drink.

The city’s tattoo culture spans that whole messy range, from the sterile calm of The Grey Ash to the mall surprise of Deja Vu. I went in for one thing and left with a dog portrait that made me cry.

I’ve already booked my next appointment. Stop thinking about it.

Highlights
Artist listens until you’re happy
Mall location surprises, then wins
Izi’s light hand, steady work
Easy booking, worth the trip
Loyal repeat customers, finished pieces

I walked into Deja Vu expecting a mall tattoo shop, which is to say I expected nothing. I left with a piece I actually like. Izi has a light hand, and the work is steady, clean, and honest. The artist listened until I was happy, which took longer than it should have, because I am difficult. Repeat customers kept coming in, and a few showed off finished pieces that made the whole wait worth it. The location is strange. The vibe is stranger. The result is solid. I booked again before I left. That is the review.

Skilled artists, welcoming vibe
Inclusive and friendly shop
Great flash sheet options
Willing to customize color
Customers return repeatedly

Dark Roots has that thing every shop claims but rarely delivers: a welcoming feel. I sat down, looked at the flash sheets, and got a piece customized with color I would never have chosen myself. The artist didn't flinch. Returning customers filled the seats, and I understood why. The place is inclusive without performing it. Skilled hands, no attitude, and a sense that they want you back. I have already started planning the next one. For a city that has many options, this one does not need to shout. It simply holds its ground.

Skilled artists with strong portfolios
Custom designs tailored to clients
Cozy, cool, badass vibe
Repeat clients planning future work

Old North State Tattoo Co. is clean, and I am allergic to dirty needles anyway. The artists have portfolios that made me feel self-conscious about my own planning. They actually listened to my vague ideas and gave me something tailored to what I meant, not what I failed to say. The mood is cozy, cool, and quietly badass. I watched repeat clients walk in, ready for their next piece, and I got why. I put a deposit down for future work. That is a commitment. I do not make those lightly.

Katie’s suggestions improved my design
Clean shop, personable service
Adam’s detail work is incomparable
Worth a repeat visit

Tattoo Archive is a small room where Katie made me feel seen. She suggested a change to my design, and I fought it. I let her try. I was wrong. The piece is better now. The shop is clean, the service is personable, and Adam's detail work is unreal. I watched him work on a line that most people would ignore. He did not. It left me with a strange respect for the craft. I will go back. Not for the comfort, but because they earned the repeat visit. Some shops grow on you. This one got me fast.

Highlights
Perfection every single time
Artists take genuine pride
Trauma-free piercing experiences
Staff become good friends
Best shop hands down

Love Ink feels like a contradiction: the name is cheesy, the work is not. I have been pierced and tattooed here, and each time it ended the same way, perfect. The artists take real pride in their craft. I watched them argue over the angle of a line, properly arguing, like it mattered. That kind of care is rare. My piercer handled a needle like a friend pushing back a panic. I left less afraid. The staff started as people I paid, and somewhere in the last year they became people I would get coffee with. That is not a complaint.

Highlights
Expert piercer, Victoria, shines
Clean, organized, welcoming space
Handles emergencies with care
Honest placement advice given
Genuine client-first service

Rowan Thruway Center is not where I expected to find a piercer I trust. Victoria is the reason I come back. She walked me through everything, step by step, with an honesty I appreciated, because I am looking for a needle and I had no idea what was about to happen. She told me when a placement idea she liked was still wrong for my body. I missed that feedback. The space is clean and organized, and I saw her handle a walk-in emergency like it was routine. That is doing the thing.

Tiny’s fine-line mastery
Consultation feels personal
Nearly painless needlework
Stephanie’s custom dog art
Multiple designs, zero pressure

Tiny at Seventh Sin does fine lines so delicate I questioned whether it was anesthesia. The consultation felt less like a sales pitch and more like a conversation I was actually part of. She suggested a few directions, and then she stopped talking and let me breathe because she noticed I was tense. A nearly painless handful of minutes later, I had a piece I keep looking at. Stephanie custom-painted my dog. I have cried twice. The shop offers multiple designs, but there is zero pressure to rush. That is the rarest part. I did not pick until I was certain.

Mitzi delivers meticulous tattoo detail
Tap mixes laughter with skilled work
Clients return for ongoing projects

Wildfire Tattoo Studio runs a clean, professional space, which is a low bar, but they clear it. Emily is the one who did my piercing. It was less a procedure and more a very calm conversation that involved a needle. Mitzi worked on the tattoo, and she is meticulous in ways that see tiny mistakes before they exist. The real delight is Tap. He mixes laughter with genuinely skilled work. I have a few ongoing projects. I keep coming back because they do not just did me. They remember what I wanted.

Quinn’s bedside manner shines
Clean shop, quick walk-ins
Quality jewelry worth repurchasing
Gentle, step-by-step piercing care

510 Expert Tattoo is the fastest walk-in I have had, and were not rushed. Quinn has that rare ability: a bedside manner that makes you feel like you are doing something intimate, not an oil change. The shop is clean, straightforward, and the jewelry they stock is quality enough that I have bought more once already. I got a piercing and the guidance was step by step even for a second, when I started to shake. The care was gentle, the pain passed, and I have a plan to return. Easy is underrated. They make it feel easy.

Highlights
Kindest, most professional artist
Unique black and grey work
Calm, anxiety-free experience
Patient with endless questions
Impeccable cleanliness and service

The Grey Ash is not a tour spot. It is quiet, careful, black and grey linework that does not push color to prove anything. The artist is the kindest I have met in this industry, patient when I asked questions again and again, never once showing the fear behind his eyes. The room stays calm. I went in anxious, and came out with a piece I actually love. The cleanly exudes calm from present to end. I cannot remember another tattoo visit where I finished and was not exhausted. That is rare.

Highlights
Huge portions, fair prices
Fast, attentive waitstaff
Upstairs tattoos and piercings
Loud music, not for elders
Bar backs save the day

Tattooz & Booz is downstairs for the food, upstairs for the needles, and I need to be honest with you about my evening. The portions are enormous, and the prices are fair. The waitstaff came by enough if actually, they came by enough, it was fast. I went up for a tattoo and got a piercing at the same time. The music is loud. If you are old, like I am, you will miss a few instructions, but the placement still worked. The bar backs actually saved my night when I spilled my drink, and the tattoo is solid. It is chaos in here.

Highlights
Clean, professional, and friendly
Skilled with first-timers
Fair prices, quality work
Worth a return visit

Blackout Ink is the clean, professional stop I did not know I needed. The artists handle first-timers like prodigies, which I suspect they are. Pricing is fair, a word I rarely use without sarcasm. I got a simple piece. It was done well. The parlor has that quiet confidence, no upsurge, just a table, a set of skills, and an open door. I am going back. The first visit settled something in me. It is not the wildest place in Winston, and that is precisely why I trust it.
